I built the one on this link and works very well. great demo and instructions.
What did you use for runners for miter slot?
I used hdp plastic that was about 3/8th thick. Very common plastic and had some lying around. easy to cut and use. Make sure you wax the bottom of sled and top of your table. You will appriciate the slide back and forth after because the sled can get heavy. paste wax works great.

On mine I used two oak runner's and wax them also do not trim flush with table top make them about 1/8 thicker so the sled rides just above table for less friction.Makes a huge difference.
I have a couple of them, Mine are just the single fence type. plywood with one oak rail that runs in the slot to the right of the blade and it has a oak fence. Make one with the fence toward you and the other one with the fence away from you both of mine run in the slot to the right of the blade. You are a little bit limited with the one with the fence toward you if the material is to wide, the slide table will want to tip of the saw table. But the one with the fence away from you will handle larger material because the fence part is closer to the blade and the material is now toward you . I have used the 2cnd one to cut off the bottom of new doors for the house. they are a great jig to have.
Excellent addition to your saw. You can find plans online. Once you have one you will be able to even cut perfect circles with your table saw.
I have pics of a muffin stand that i cut the circles out with the table saw and slide table. posted ( nov. 2 2010 )

This `was really impressive my friend.
