If you use this outlet you do not have to use a fuse with the lamp. This outlet is now available at the orange big box store or online.

The iron pipe fixture is actual 3/8" iron pipe. What I did was use a piece of lamp all thread and I ran it through the iron pipe, flange and wood top, The lamp socket is at the top of the all thread and at the bottom a nut holds everything in place. You could also epoxy to glue the lamp socket to the iron pipe coupler at the top.
You can get the fuse from mouser or if you get an all usb outlet you don't need a fuse.

Not in there list on the internet. I have amazon prime so could go that route . Haven't been in the store to check might have it there. They did have the double with single outlet.
I got my four usb block on Amazon, but since I have seen them at my home depot in Woodbridge Virginia. I bet HD will ship it to your local store for free, since they do not have it yet.
I was intrigued by your lamp design but looked at home depot site could not find the 4 usb block ?
